POSITION: PERMANENT SUPPORTIVE HOUSING CASE MANAGER

SUMMARY: This is a full-time position for case management in the Permanent Supportive Housing (PSH) program working with clients who are homeless at the time of program entry and have at least one type of disabling condition. The PSH Manager provides ongoing supportive services to program participants, assists individuals in the process of obtaining and maintaining housing, offering relevant community referrals, providing ongoing advocacy for enrolled clients, and completing documentation as required by PSH guidelines. Duties: complete required intake paperwork, individualized housing plans and progress notes; provide timely documentation of all work that meets Homeless Management Information System and program standards; provide service coordination/case management at least monthly with each client; arrange, coordinate and monitor services on behalf of client; provide self-help and living skills training which are focused on stabilizing individuals in their current housing situation; assist in evaluating and defusing crises as they occur; ensure that human and client rights, rules, laws and requirements are met; and ensure that all program rules and standards are met. Ideal candidate will have thorough knowledge of case management, community resources, and records management; occasional flexibility in work hours; ability to work independently and within the team model; communicate effectively; remain organized and detail oriented with persons and material; establish service goals and objectives; relate to individuals and families under stress; demonstrate understanding and appreciation for cultural diversity among the population receiving services; is computer literate in word processing and use of a database; considerable experience in case management, or similar experience, providing direct services; completion of a Bachelor’s degree in social work, social services, sociology, psychology or a closely related field or any equivalent combination of experience and training which provides the required knowledge, skills and abilities; and possession of a valid driver’s license and acceptable driving record.
